A 68 year old woman with a double knee pain for a period of 5 years. 1 years ago in our hospital, diagnosed as "double knee osteoarthritis, knee arthroplasty for concern, good functional recovery, for the treatment of left knee readmission. Physical examination: the left knee joint swelling; local tenderness; knee flexion deformity, and -20 degrees, 85 degrees flexion; X-ray showed severe degeneration of left knee joint, joint space disappeared, joint peripheral osteophytes, severe degeneration of patella. Varus of the knee joint, medial tibial plateau defect (Figure 1). Full preoperative preparation was concerned left knee replacement, removal of osteophyma in operation, the balance of soft tissue, intraoperative assessment of bone defect of tibia, medial platform for uncontained bone defect, namely aori type T2 type. Reconstruction of bone defects with autogenous bone, screw fixation, and rotational hinge knee prosthesis. Postoperative X-ray showed satisfactory position of artificial knee joint, (Figure 2)

In Figure 1, the left knee joint line is lateral, left knee joint degeneration is serious, the joint gap disappeared, joint peripheral osteophytes, severe degeneration of patella. The knee varus was obvious and the medial tibial plateau defect
